PURPOSE:
Through partnership and collaboration, Building Bridges serves as a community-based ally that disseminates information through training and direct service with families in need.

STAFFING:
Supervising Clinical Case Manager, 3 Clinical Case Managers
TARGET POPULATION AND ACTIVITY: 
  • The program serves 50 Westchester families annually who have at least one family member in substance abuse treatment and whose child(ren) are determined to be in need of services.
  • Each Clinical Case Manager works with 10 families at a time, and will continue providing services to each family until all identified needs have been met.
  • Any substance abusing client in outpatient treatment with children can voluntarily be screened by their counselor, and if the screening indicates a need, the entire family can be referred to our program for clinical case management services.
HISTORY:
Building Bridges is a 5-year federal grant from the U.S.Department of Health and Human Services Administration, Administration for Children and Families. The grant was awarded to the Department of Community Mental Health who will have oversight of the project. The partners include the Department of Social Services, Lexington Center for Recovery and Andrus Children's Services. Lexington Center's role in the grant is to provide long term clinical case management services to at least 250 families over the 5 year period.

  • recovery coaching;
  • 24 hour availability
