To preserve family stability and to prevent recurrent child abuse and/or foster care placement for chronic substance abusing families.
|
|
|

TARGET POPULATION AND ACTIVITY:
|
|
|
- The program serves 60 families annually living in either Yonkers, Mount Vernon or New Rochelle who are involved with Child Protective Services (CPS) and whose children are at-risk for out-of-home placement.
- Each Intensive Case Manager works with 7 to 9 families at a time for 9 months.
- Clients are screened and if appropriate referred to our program from Child Protective Services (CPS) or Preventive Services (PS) Units at the Department of Social Services in Yonkers and Mt. Vernon.

The Families First Intensive Case Management Program began as a 2-year demonstration project funded by the New York State Office of Children and Families Services beginning in December 1997. The program now operates under the auspices of Westchester County's Department of Social Services, Mandated Preventive Services.

SERVICES:
|
|
|
- A comprehensive family assessment;
- Home visits at least 2 times a week initially, then weekly and as needed;
- Linkage of client to appropriate services;
- Advocacy for families within each service system;
- 24-hour availability.
