To receive your heirloom quality teddy bear, complete the following instructions:
1. To order a Sympathy Bear Gift Certificate, go to step 7.
2. Decide on what article of clothing you would like transformed into a
bear. (all types of fabric are acceptable except those with a lot of stretch and silks)
3.
Fill out beary special information section below.
4. Complete and submit the order form.
5. Mail your special clothing, together with a check or money order to:

Clothing items to be mailed through your US Postal service should be insured. Costume jewelry or other items relating to the special event or person can also be included in your mailing.
6. In about 4-6 weeks you will receive your heirloom quality teddy bear. This bear will help you embrace your memories for years to come.
7. To order the Sympathy Bear Certificate:
a. Complete and submit the order form. If you would like the certificate mailed directly to the recipient, please complete the “Shipping” information section.
b. Mail your check or money order to:

c. The certificate package ( which includes a sympathy card, gift certificate,mailing instructions, and a pre-paid mailer) will be mailed upon receipt of your order.
