within the Department of Veterans Affairs


HOSPITAL MEDICAL MISTAKES
(March 30, 2005)

The American Association of Critical Care Nurses (AACN) recently released a study addressing hospital medical mistakes. In this study, many hospital employees admitted witnessing co-workers take dangerous shortcuts or exhibit poor clinical judgment, but they kept quiet. This study states this "culture of silence" has to end in order to ensure patient safety and outcomes, as well as improve employee morale and retention.

The study states hospitals have to implement these six standards to change this culture of silence.

  • Nurses must be as skilled in communication as they are in clinical matters.
  • Nurses must pursue and foster exceptional collaboration.
  • Nurses must be intimately involved in making policy, directing and evaluating clinical care, and leading organizational operations.
  • Nurse staffing must ensure an effective match between patient needs and nurse competencies.
  • Nurses must be recognized for their value to the organization, and must recognize others for the same.
  • Nurse leaders must fully foster a healthy work environment, and engage others in its achievement.
